Holter monitor billing starts with how long your patient wore the device. A 14-day patch billed as a 48-hour Holter goes out on the wrong code. We code each monitor from the recorded wear time, and we sort out the vendor's part.

A Holter worn up to 48 hours bills as 93224. It covers the hookup, the analysis and your read. Split, it is 93225, 93226 and 93227.
93224 and 93227 on 1 claim bill your read twice, and the second line denies. The code follows the recorded time, not the order. A Holter removed at 20 hours is still 93224.
Past 48 hours, 3 to 7 days is 93241 global. Its parts are the hookup 93242, the analysis 93243 and the read 93244. 8 to 15 days is 93245 global, split the same way into 93246, 93247 and 93248.
With a vendor patch, the vendor's IDTF bills the analysis, 93243 or 93247. You bill the hookup if your staff applied it, and your signed read. A global code on your claim overlaps the vendor's, and it denies.
A 14-day patch pulled on day 5 bills from the 3 to 7 day family.
