Device check billing on a calendar

Pacemaker check billing: in office and remote

Pacemaker check billing pays by device type and by how often you can bill it. An in-office check is coded by device and by whether settings were adjusted. A remote check is 1 claim per 90-day period, and we run that calendar for you.

In-office interrogation and programming

Interrogation is a device read with no settings changed. It is 93288 for a pacemaker, 93289 for an ICD and 93291 for a loop recorder.

Programming is when your physician adjusts settings to test them and select final values. Pacemakers are 93279 to 93281 by lead count, and ICDs are 93282 to 93284. Programming includes the interrogation, so you bill 1 or the other for that device.

A CRT-D is a multiple-lead ICD, 93284. Coded as a dual lead, it pays less. We take the lead count from the implant record and check the signed report.

02Remote

Remote monitoring: the 30 and 90 day periods

Remote pacemaker review is 93294, and remote ICD review is 93295. Each covers up to 90 days and is billed once per period. The technical part, 93296, belongs to whoever runs the monitoring.

Loop recorders run on a shorter clock. 93298 covers up to 30 days, billed once per 30-day period. On Medicare claims, G2066 is the technical part.

We track both clocks per patient and per device, and hold each claim until its period closes.

03Denials

Loop recorders and remote denials

Loop recorder insertion is 33285, and removal is 33286. The implant diagnosis, such as syncope or cryptogenic stroke, carries to every remote claim.

5 things commonly deny your device claims:

  • EarlyA remote claim sent before its period closes.
  • OverlapAn office interrogation inside an open remote period.
  • BundlingAn interrogation billed with programming for 1 device.
  • Lead countA CRT-D coded as a dual lead ICD.
  • DiagnosisA remote claim without the implant diagnosis.

We check each one before a device claim goes out. When a denial still comes back, we appeal it with your note.
