What we bill: every cardiology study, in your office or the hospital, and nothing else. Each page below covers one study: the codes, the denials we see and what we check first. Start with the study you're being denied on.

These studies have two parts, the scan and the read. In your office you own both and bill the global.
On a hospital scan you bill the read with modifier 26. We check the component on every claim before it leaves.
These studies pay by who supervised, how long the monitor was worn, and time since the last check. A remote device check sent a day early denies. A 7-day patch sent with a Holter code underpays.
Office visits are leveled from the note, with G2211 on the Medicare visits that qualify. G2211 doesn't pay on a visit billed with modifier 25, unless the same-day service is a preventive service, a vaccine or an annual wellness visit. A same-day study is billed with the visit when the note supports both.
In the hospital lab, diagnostic cath carries modifier 26. PCI and ablation have no 26 or TC split. We check the prior auth and every add-on against its primary code.
Code-level guides for your biller or office manager. Each one links back to its study page above.
If your study isn't listed, call and ask. If a cardiology practice does it, we bill it.
We review them by study and by payer, free, with no strings.
